    2. Mayor’s Permit / Municipal License
    3. SEC/DTI Registration
    4. Business Permit
    5. Detailed Location sketch of the Sales Office and Warehouse
    6. List of Products/Items (Brochures and Catalogues) if applicable
    7. Company Profile including list of clients

    II. REQUIREMENTS FOR LOCAL SUB CONTRACTOR

  • Kindly fill up this Subcontractor's Accreditation Form and upload the following attachments to Subcontractor Accreditation Attachments
