  • Mayor’s Permit / Municipal License
  • SEC/DTI Registration
  • Business Permit
  • Detailed Location sketch of the Sales Office and Warehouse
  • List of Products/Items (Brochures and Catalogues) if applicable
  • Company Profile including list of clients

  • II. REQUIREMENTS FOR LOCAL SUB CONTRACTOR

  • Kindly fill up this Sub-Contractor Information Sheet and send the following attachments to: Email: info_subcon@sldcc.net

  • List of Attachments :

  • BIR Form 2303 Certificate of Registration
  • Mayor’s Permit / Municipal License
  • SEC/DTI Registration
  • Business Permit
  • Detailed Location sketch of the Sales Office and Warehouse
